DOG CARE RELEASE OF LIABILITY

DogJax boarding facility provides indoor open play yard, outdoor play exercise, socialization, training and boarding and grooming activities. Please check each box indicating that you understand and agree to the following: *
Dogs can incur scrapes, scratches, bites, sprains, strains, broken bones, cuts and other injuries or even death.Dogs can be exposed to communicable viruses and parasites such as Bordatella, lice, fleas, ticks and other diseases and parasites even with medication and vaccination.Your dog must be current on all vaccinations, including Bordetella and any anti-parasite medication. We require up to date vet information.Your dog is fit and healthy enough for play, exercise, grooming, and boarding.If Daycare dogs are not picked up before closing they will be automatically boarded at the expense of the owner.Discounts on services are not offered during holiday or peak season.We may provide treats, chew toys, and other edible items for your dog. If you do not wish your dog to have any of these items let us know.Any toys, beds, leashes or equipment that you bring for your dog may be lost or destroyed. DogJax will not reimburse for lost, damaged or broken equipment.If your pet requires medication, the issuing veterinarian provides all information for administration.DogJax has permission to take necessary medical, or other, action should an emergency arise during your pets stay. Owner will cover this expense.You must disclose if your dog has shown ANY signs of aggression towards other dogs or people at any time, for any reason.There are occasions when the dogs are not supervised during non-business hours (7pm – 7am).Grooming may expose pre-existing health/skin problems for which DogJax cannot be held liable. Dematting/thinning, grooming of matted hair may cause patchiness and hair loss. Shaving of pets may cause a dramatic change of appearance.Cancellation policy includes 7 days notice prior to dogs overnight stay and 14 day notice during holidays, 24 hour cancellation for grooming and training appointments.Owner agrees to pay all fees and incurred expenses before pet will be released to go home.Dogs must have tags that identify the dog by name. If not temporary tags will be put on dog by our staff.Dogjax is not and cannot be responsible for any pregnancies of dogs that result in the co-mingling of non spayed females and non neutered makes. If your dog is not fixed than take other precautions or make other arrangements for your dog.

Liability Agreement *
Owner agrees to release DogJax LLC and hold harmless its owner and employees from any and all liabilities of any nature. I agree to take complete responsibility for the actions of my dog, and myself before, after, and during class. At no time will the instructor of this class be liable or responsible for the actions of me, my dog or any other person who accompanies me to class.I have read and agree to the Liability Agreement.
